Download the appropriate Verification form below. Make sure the form is for the correct financial aid year.

2024-25 Academic Year (July 2024 – June 2025)

**Students must reapply for financial aid every year. 

2025-26 Academic Year (July 2025 – June 2026)

To complete the verification form, use one of these tax retrieval methods to make corrections and/or verify your FAFSA information:

• Request a Tax Return Transcript directly from the IRS
• Or provide a signed and dated copy of the filed tax return.

Make sure to submit all of the tax information for you and your spouse if married, or you and your parent(s) if dependent.
