Please download the Participant Reimbursement (PR) form.3. On the PR form, select the items that you are needing, with the amounts. Please include the book names, along with your justification under the section labeled “Mandatory”.4. If amounts are not yet available on the bookstore, please leave amount section blank on the PR form. You will enter 0.00 on the CSA question regarding the costs of books. The PR form will still need to be completed and submitted.5. Complete PR form and save edits6. Upload your completed PR form below, this is mandatory (without this we are not able to provide assistance).
