SimMan 3G Trauma Training: Programming compounding scenarios

Trauma Training 1Big Bend Community College and Title V brought in Kelly McKeever, a recognized expert in medical simulation, to provide a two-day intensive training on how to run and design emergency scenarios using BBCC’s high fidelity simulation equipment.

Due to the tremendous guidance by McKeever, the trainees were able to tackle of the task of becoming independent programmers and operators of BBCC’s high fidelity trauma simulator. Even though most of the participants have had prior experience in medical simulation, this was still a challenging undertaking because there were literally hundreds of new training elements that were addressed, taught, and repeated throughout the training.

 

Below are some of the most memorable outcomes of the training:

Controlling airway obstruction. Trainees were able to practice programming BBCC’s SimMan Trauma to clench his jaw; swell his tongue and/or esophagus; and respond to a head-tilt-chin-lift needed to clear the airway for CPR.

Hemorrhage or severe bleeds. Several hours were spent working with the SimMan Trauma’s bleeding capabilities. Trainees learned how to produce bleeds from both veins and arteries located throughout the simulator. Some of the bleeds were large (severed leg or arm) and others were smaller (surface level stab wounds).

Circulatory system. SimMan Trauma has a series of ducts, valves, ports, and tubes throughout his body that circulate fluid. This system is used to have the manikin simulate secretions such as sweat as well as urination. Trainees had an opportunity to program SimMan Trauma to cry, sweat, froth at the mouth, have a runny nose, and produce discharge from his ears. Additionally, (and this falls into two categories), the trainees were able to start an IV (while verifying placement by getting the flashback of blood), push medications, and program the manikin to respond to the treatment.

Integrate the use of authentic medical equipment. One of the reasons that high fidelity simulators provide excellent training is because healthcare professionals use actual medical equipment. Part of the advanced training that the SimMan trauma trainees received was learning how to program the SimMan Trauma to respond to the actual equipment. For example, trainees spent ample time programming the simulator to react to a megacode situation (for example, the shock from a defibrillator, intubation equipment, vital sign obtainment, and so on).

Compounding scenarios. Trainees also spent time setting up sequence of events that would trigger other programs to start. For example, certain treatments by healthcare professionals (such as the administration of morphine) would trigger a trend in the manikin (that the Simulation Technician would have preprogrammed) to respond to the medication by changing some of the physiological expressions of the manikin (such as vitals).

Overall, the training was a fantastic experience that produced six independent SimMan Trauma operators and programmers. This training will benefit the Moses Lake community by providing even more realistic training opportunities for the healthcare and emergency response professionals in our area.
